Freshman Admission Requirements:
Admission Requirements - Freshmen: 1. Completely fill-out the MMCC Application Form 2. Form 137 (First to Third Year grades) with the official seal issued by the High School Registrar or the Principal (photocopy only) 3. NSO Birth Certificate (photocopy only) 4. Character reference from the High School Principal and/or Guidance Counsellor 5. 2×2 ID pictures (2 pieces) with your complete name printed legibly at the back NOTE: Only duplicate copies of the documents are needed for submission, however, original copy should be presented for verification purposes.

Transferees Admission Requirements:
Admission Requirements - Transferees: 1. Completely fill-out the MMCC Application Form 2. Form 137 (First to Fourth Year), Diploma (High School) and Transcript of Record with course description 3. NSO Birth Certificate (photocopy only) 4. Character reference from the High School Principal and/or Guidance Counsellor 5. 2×2 ID pictures (2 pieces) with your complete name printed legibly at the back • NOTE: Only duplicate copies of the documents are needed for submission, however, original copy should be presented for verification purposes.

Foreign Students Admission Requirements:
Admission Requirements - Foreign Students: 1. Completely fill-out the MMCC Application Form 2. Transcript of Records / Certificate of Graduation with the official seal issued by your High School (photocopy only) 3. Birth Certificate (photocopy only) 4. Character reference from the High School Principal and/or Guidance Counsellor 5. 2×2 ID pictures (2 pieces) with your complete name printed legibly at the back 6. Passport 7. Visa • NOTE: Only duplicate copies of the documents are needed for submission, however, original copy should be presented for verification purposes.
